FPIC offers several options for premium payments. These options include:

  • One-time annual payment
  • Quarterly payments
  • Nine payments throughout the policy term. Requires 20% down payment and 8 installments of 10% thereafter.
  • Credit card (Visa or MasterCard only). First installment must be made by check and credit card may be used thereafter.

To ensure that your payment sent via regular mail is applied promptly, it is important to use the remittance slip and pre-addressed envelope that is included with your installment invoice.

Please note that the first installment must be made by check and your credit card may be used after the first check payment.

If you need to send your payment by overnight mail, please use the remittance slip included with your installment invoice. The address for payments made via overnight mail is:
